One singular merkle tree with it's root node hash being regularly published in bitcoin's chain - that approach is in no way scalable, yet was quite instrumental in how keybase works. Every action that changes keys or identity of the account is logged in the global merkle tree, and the publishing if the root hash ensures that the tree cannot get forked.

As for device system, essentially, each device has a separate EdDSA key, end everything sent to one person gets encrypted for all of their devices. If new devices get added, existing ones can re-encrypt everything they know about to the new ones. You get screwed if you loose all of your devices, but they do recommend to get paper backups keys which can also act as devices - it's just a bunch of words, and it's a decent recovery solution.
