Your instance seems to be back on v4.2.2 -- which, although almost new, is still one release behind
Don't recall what issues people had with v4.2.2 but generally I recall a pretty quick update to the current release v4.2.3
2) You say Safari on iOS
That's almost exactly what I have on my iPhone 14, Safari, but that's nothing like what I see when I test post with a test image and alt text
