Also, of all platforms supporting custom emojis, I've only seen them being rendered this small on Mastodon. When I first joined fedi on a Mastodon server and found the custom emoji feature, I thought it was completely pointless because I couldn't make out what any of them were.
If the majority of custom emojis are fine everywhere but on Mastodon, it's Mastodon which needs to adjust. But Eugen has expressed regret at introducing custom emojis, so it makes sense why things are like this.
