I had a similar experience. At one point my lightning was removed and another instance my relay list was changed but after investigating what can happen is a client your using doesn’t fully load and then you update your profile or actually hitting follow on someone is the same event as a profile update and suddenly you unknowingly sent an event with a missing lightning or changed relay list. It probably doesn’t mean your key is compromised
