Signal is working on the ability to set nicknames and add notes to contacts! Notes and nicknames will only be visible to you and will be end-to-end encrypted.
This feature is currently available in Signal for Android v7.2, but it will likely come to other platforms soon. Please note that this feature is currently under active development, so its design and functionality is subject to change.
