I just wasted 3 hours with this vaccine affidavit!
The lady claims I filled it out wrong. I think she needs read the sentence slower.
Please help... Between my name and the notarys name, which names go in which spots ?
In the end, I got it filled out the way she wanted and now the day care will accept my child. She said she called 10 other schools and they all agreed with her.
