If there really aren't assassins, but are determined reformers convinced enough hallucinogens will permit Blofeld to rearrange his brain in a less antisocial way, you could plausibly have a lot of fun with the nature of narrative reality and casualty. (And possibly get hallucinated Bob Howard in there anyway.)
