This was circa 2008, I did a residency rotation with an ENT that specialized in the neurological side of things. He would identify the frequencies and the patient would fill up an iPod with music they liked. He would mail it to some company and they would remove the relevant frequencies from the audio files.
When the patient listened, their tinnitus tones would slot into the gaps. He said most patients couldn't afford it. I don't know if it still exists. Homebrew solutions might work.
