I was glad to see Baal pop up as terrorised because it was a nice break from grinding Mephisto. I've been nailing down the optimal strat for Meph runs though. I tried doing all the chest and enemies in main area and two side rooms, but got two extra items drops in 25 runs and they were ones I already had, none from chests. Then mixed it up and tried different combos. TL;DR just do the unique group at the bottom, Meph, the unique on each side room, and other unique monster groups if they spawn.
