It's cathartic to imagine violent retribution, but it's wrong. First, because it's unjust, and secondly because it doesn't work. System failure has to be handled at a system level. Picking a scapegoat, no matter how senior, doesn't do that because it doesn't distinguish between malice and an impossible job.
