A mix of PoW, bayesian spam score, and other metrics like looking at who follows the user and the distance of their followers from you (WOT-ish measure) will probably get us a long way. These are not new technologies. The bayesian spam checker running on my email server filters 99% of spam and it is a very old piece of software with a simple implementation:
