What about if you orgasm in the night, now that you don't have a sexual partner to regularly release with? or does this feel completely different to you, I mean, can you feel the contractions and pleasure, or just pain? Seems like the full bladder may trigger sensations of pleasure for you, but then it shouldn't be hurting, maybe this is normal for you as you've had it so long. I've not heard you mention children so assuming no pregnancies have brought this on.
Maybe a trip to the gp, to check on things in case something has changed, but odd that you've been having this since your twenties.
