It seems to me... If a patient comes in with an extremely painful condition... And, you know, for a fact, because of nifty diagnostic stuff that we have, that you've used on the patient... And, you know ABSOLUTELY that the patient has not only been diagnosed with the extremely painful condition, but REPEATEDLY admitted to hospital for the extremely painful condition...

And the patient presents with a flare up of the extremely painful condition, and is showing clear CLINICAL SIGNS (not just symptoms, but actual signs) of both a flare up of the extremely painful condition and clinical signs of extreme pain...

I don't really care if the attending physician claims the patient is an addict and is drug seeking. I think the patient is in pain and should get medication for pain.

You know what? Even if someone has a substance use disorder, they can still have extremely painful conditions that need treatment and you aren't going to fix an addiction by refusing to medicate someone who is showing clear physical signs (complete with lab changes and diagnostic imaging images) of having an extremely painful condition. In fact, someone who's habituated to opiates is going to need more to get pain control.
