AIUI: The ECS on a plane only maintains cabin pressure and expects oxygen partial pressure to be acceptable. But this means ppo2 can drop if o2 flow slows enough and ECS won't even know it, then everyone passes out and the plane crashes and nobody knows what caused it, just that it went unresponsive
