Discovering one of the (many) problems with being having to change doctors this year. Filling out the pre-visit survey, it appears I’m going to need at least three visits in order to get my records correct. There were multiple transcription errors (mostly wrong dosages for prescriptions), duplicate medications listed, other medications missed entirely, symptoms which were related to meds I’m not taking any longer… and everyone was so rushed, that the second visit didn’t get them all corrected.
