I just finished reading Geek Love by Katherine Dunn. It was published in 1989. It's about a carnival family who creates their own freak show by having the mother deliberately ingest teratogens to have deformed children. They feel superior to "normies", people who don't have deformities. One child, billed as Aqua Boy, becomes a megalomaniac who starts a cult where people voluntarily amputate their limbs to be like him.
