well man the issue with IUDs is that even if research suggests that it is safer than before, it isn't the same with individuals who often face side effects from chemical like hormonal inbalances (and long term effects of chemical IUDs) and physical rupture which requires more surgery to fix these issues

So there isnt exactly have been a non risk method which also requires significant healthcare costs, for the average sex worker it would mean taking significant debts which they would have to earn sufficient wages which may take them years to cover up the costs, not to mention it would be harder for them to find job opportunities considering companies outright blacklist individuals involved in sex work
