Insurance relies on trust, there is 0 point in paying insurance payments to an organisation you don't trust in paying out to you in a time of need. When a doctor asks an insurance company for the payout on a treatment, they could be put through to someone who has no knowledge of the treatment and no basis to refuse it.

All medical treatments should already go through an MDT before being decided on, there's no need for further disputes. An insurance company could have professionals on hand to verify the need of treatments (like they used to), or better yet, just.. trust doctors.
Accountability matters.
