I used to have a Word document for the story, and then a separate Docs or Word document for the world-building, outline, and general notes.
Since I bought Scrivener, I've taken to keep everything on the same project, using the app's folders and files to separate the manuscript from everything else. Revisions are also divided by the app's colour system.
And I guess that's pretty much it x)
