I am sure there are many other examples, but last year I was hospitalized. I went to the ER with a heart issue and they admitted me to the hospital. In the hospital they performed a procedure that set things right. After the fact (at least I assume it was after the fact) the insurance company denied the entire hospital stay as "medically unnecessary." Although they did pay for the procedure (go figure??) Fortunately, because of the network terms, the hospital could not bill me, they had to fight it out with the insurance company.
Still strikes me as completely insane.
