My somewhat informed take is that it mostly “doesn’t work like that” given the relay model. It isn’t a great structure for secure, private messaging. Why? Mostly because notes are all public, DMs too - just with their content encrypted. Being public means anyone can see and save them forever. And given “forever” timescale the encryption would need to be… well… infinitely secure.
