When you ask primal "how many followers do i have?", they look at value in their database.

When you ask damus, its something like: they look at which relays you have configured in your app then they go and ask each of those relays "who is following tanel?" As the answers come in they increase the counter. Damus also has a built in relay so some of this information might be in your device already and they get it from there but also check with relays for possibly more current data.
