Mk 39 thermonuclėar bõmb no. 434909 accidentally dropped from a B-52 on Goldsboro (North Carolina) in 1961. The plane carrying the ordnance broke up in mid-air, dropping its nuclėar payload in the process
Information declassified since 2013 has shown that one of the bõmbs was judged by engineers at the time to have been only one safety switch away from detonation.
There are still many "broken arrows".. (missing nukes).
