Just a lot of experience, mate. I've been shooting photos professionally since 1997.
With that experience, you know how to manage and handle light, what cameras are capable of, what shutter speeds and apertures to set, where to place the flashes, what settings to tweak them to, etc, and can do it relatively quickly.
Mind you, I still screw up... a lot. I always seem to mess up DoF especially.
Here's the time frame for this shot:
a) I want an after dinner espresso
b) there's my Canon 6D with the flash mounted. There's one of my other flashes on the dining table. Grab that too.
c) put the remote flash about 1.5m away on the counter, facing up. Make sure everything's turned on and paired up.
d) prep my espresso shot. Don't even bother wiping down the machine much lol)
e) Get the shot loaded in the machine, pick a nicer cup than I normally would have used... and oh yeah, a glass saucer would be nice too.
f) pull the shot, grab camera.
g) direct my flash, frame the shot, start shooting. Check first, light's okay, keep going.
h) shot's done, put camera down, walk back to dining table and enjoy my espresso ;)
Then much later, remember I took the photos, so used my smartphone to transfer the RAWs to my phone, and upload it.
