They can use your DM:s, impersonate you and other bad things.
The stakes aren't as high if you're compromised on nostr but I would still say that its still a great way to bring more autonomy and sovereignty to the individual as the foundational principles of social media as a protocol is to put the individial first. Especially for users running their own node, you keep and control all your content and social graph. If compromised, you have to start over with new keypair building those connections and a new history.
Not too much of a big deal now, but imagine in the future, important public people or entities running nostr. Key management will be important.
