IMO we need a Patients' Bill of Rights like for air travel. If your flight gets cancelled, the airline has to put you in a hotel, etc.
If I'm late to a doctor's appointment, they can cancel the appointment and charge me a no-show fee. But I can sit in the exam room for a half hour, hour, or more past my appointment time with no recourse. I'm lucky my job is flexible; I feel bad for people under (perfectly normal) time constraints.
