I don’t know whether Primal specifically does anything differently but in general a Nostr client will just write a kind 1984 event to the relays when a report is made. It is then up to clients or relay operators to do something with these events. Some relays and clients may use these events as part of their filtering but a lot don’t.
I don’t think there is really any way to stop someone from following. The best you can do is block so you don’t see it.
