My impression is that many people either don't know the followers-only option exists, or don't understand what it does. There would have to be some UI changes that make it more obvious and more readily understandable if you wanted people to use it as common practice.

But I can envision a lot of cases where you would want wider visibility while lowering the potential for negative piling on. I don't think Mastodon, etc. posts are exactly equivalent to a webpage socially, even if they are technically the same.
