I'm not sure. Be it nutzaps or old-school zaps, at some point dollars, euros and friends need to cross the border into lightning land.
Would be great if Nostr had it's own peer to peer exchange, small amounts only, but I don't think that's feasible either due to payment concerns.
Bit of a head-scratcher. Feels like an arcade where the games are a lot of fun but only the staff have coins.
