After that, you always have the option to pay for it out of pocket for I think 18 months. But it's super expensive, like $1,000/month, so most people try to get a new job within a few weeks to avoid having to pay for it. If you choose NOT to pay for it, you are penalized when you do your taxes. It's a helluva system :(
