I've always found the US 9/11 Commission Report to be a textbook example of a fundamental problem with US politics. Thirteen sections, fifty-five subsections, three appendices and 567 pages. It summarized 2.5 million pages of documents, interviews with 1,200 individuals in ten countries and including most senior officials from both the Clinton and Bush administrations.
It only had a vague paragraph hinting at the reasons for the attack. People don't want to know.
