It's usually opportunistic fungal infections, tho there are a small number of documented cases where syphilis was the direct cause of death. Untreated syphilis is more like leperosy than anything else: won't kill you, but if untreated, might progress to the point where u wish u were dead (in the era where treatments for advanced syphilis did not yet exist)
