Sometimes the issue is kernel-adjacent.
Back some time ago there was a three-way problem with the kernel, the USB driver and certain USB hardware.
Certain USB chipsets would randomly disconnect USB storage. It appeared that the port would go into low-power mode and then the filesystem would time out. There was a setting to never go into low power mode, but it still happened.
I even filed a bug report on it.
The kernel internals never showed a changelog that referenced a fix, but eventually it started working.
And the bug is still officially unfixed. But it works, so no one cares.
