Those surprises can and do break the bank.
For around the last decade, when I sign my name, esp digitally, I always write, 'only consent to in-network providers'.
I had an er neurological consult once, pointed that out after I got the surprise bill, and it got wrote off.
Not sure if it would hold up in court, though.
