No, it's only a write restriction to your relay. Only people that you follow, can create a new post using your relay. That means that I can add your relay to my relay list, knowing that your relay blocks spam.
It's just a very simple anti-spam relay.
You could have setup a stirfy relay with plugin's and filters to do the same job.
