Do both the server and the iOS device have a valid default IPv6 route.
Also thinking about the packet flow do the sent and reply packets follow the same path or is there any possibility that the server will try to reply via a different interface (if multiple interfaces on the server as implied)?
ULAs are not usually the preferred source address selection for the OS if there is a GUA or an IPv4 also available from the same host.
