I manage a small communuity college vocational training location, and deal with this stuff all the time despite having access to a 'master' locksmith. Some favorites:
Roughly half the doors remain unlocked when opened with a key, half revert to locked (they should all revert to lock so classrooms can be locked in active shooter situation)
Deadbolt hardware mounted improperly (like above)
Pinned doors being held open using clamps due to pin mechanisms being stripped due to incorrect pins being provided
Patio sliding glass doors with the screens mounted backwards screen doors inside the glass doors
Part of the issue in my case is that the building is leased so there are arguments over fiscal responsability. But it is mostly apathy to doing things right.
