A problem that I really want someone to solve: practically minimizing code review effort in a way that also minimizes risk.
If I look at where new code spends the most time, it is in review - not because the review takes a long time, but because it takes a long time for someone skilled enough to have time to review it.
Adding more people into rotation is the obvious win, but there are also real costs involved there that are not always practical.
