Titles like "doctor" and "general" are lowercase when used in text and quoted speech UNLESS they stand in for or precede a person's name. Then, they are capitalized. (They are also capitalized when styled as an abbreviation.)
We talked with the doctor after the surgery.
We talked with Dr. Jameson after the surgery.
The ONE EXCEPTION to this that I know of is: the Doctor, that Gallifreyan gallivanter. The D is always capitalized when referring to this individual.
