A lot of alcoholics hang on and endure treatment for awhile. Maybe they're hoping for a liver transplant, although that can take years. Maybe they're hoping liver and kidney function will return, which it sometimes does. Sometimes it takes a lot of suffering before people give up and accept end-of-life care. They may not be ready to go or they may think of it as suicide.
