Usually the most vulnerable are those hurt the most - poor/elderly/sick/children/wrongfully convicted/easily indoctrinated w/propaganda/etc - which makes it hard/impossible to act against that regime or escape it or even realize there is an alternative - yes it's that grim for some.
It's easy to put everyone in the "bad" bucket and say fuck it but that can backfire tremendously. Heart goes out to women/youth/LGBT after Sharia-law takeover in Afghanistan.
