That's a beautiful question. The answer isn't obvious from the reading, but upon pondering, I think an answer is to have a meditative thought/prayer life that focuses on who Christ is in you, who you are in Him, and walking out that truth when something else (habit, thought pattern, false identity) comes against it.
This short message has helped me a great deal over the past couple years when walking things out. Since I don't know what "stream" you're from, as always, eat the meat, spit out the bones. :D

Oh, and I've never had a smoking habit, so it really doesn't have anything intrinsically to do with smoking. That's just the example.
Another thing I do when I have a thought contrary to Christ (e.g., self-hatred) is to start confessing the opposite truth in God: "Father, I thank You that you love me. You made me for yourself and there is nobody in heaven, on earth, or under the earth that can do anything about it. I do not judge a matter before its time and I am not the judge, so like Paul, I do not even judge myself. Thank You for guiding me into all truth through revelation, repentance, and simply delighting in who You are and who You made me to be..." and just keep going on like that.
