I feel emotions, usually in my stomach... Strong fear or sadness is always intensely painful.
Anxiety, nervousness, and things like make me feel like my heart is beating out of my chest even if my heart rate happens to be normal at the time.
For most of my life I thought this was normal... doesn't everyone feel that intense pain in the pit of their stomach? I know I've heard people describe emotions as causing that, and always assumed it was referring to what I feel... but now I don't know.
As for the empathy question... I only get confused about who's emotion is causing it if it is my wife, or when the emotion is strong enough, one of our kids. I'm still very much trying to better understand it.
