Ground ATC uses the same basic procedures and technology as airborn ATC. The difference is that on the ground there are a lot more aircraft in close vicinity, so the inevitable human errors are more serious. We'll have to see what the investigation into Tokyo uncovers. But there aren't any easy answers.
You imply that "ever busier airports" are the problem. That isn't the case. /1
