I assumed it was fake because she's supposed to be off work while recovering from *abdominal surgery* and she's sitting up *wearing jeans*.
I would expect someone healing from abdominal surgery to wear an empire waist dress or something else less likely to aggravate the situation, maybe even find a pose where the kids cover their lower half so it doesn't matter what they're wearing.
