It varies. Milos was supposed to be a character I'd rendered, but he ended up looking nothing like it, nor had the personality he was supposed to. Alex, I had no idea of either until I started writing. Yve (in the pic) started out as fun renders, then he decided he was a character and his personality came along. Maxim was just a personality until his appearance turned up in my head. (His pink hair made me laugh.)
