If I understand your question correctly, there is one fee that covers the whole transaction. The inclusion of your dust input for 20 sats makes the transaction slightly larger, which does increase the fee, but it's far less than the fee would be if you were to try to spend the dust input alone (which probably isn't even possible because it's so tiny)
