Hmmmm maybe there is a way to combat the spam in one of two ways.
I have run DNS web filters at home now for a long time. I used to write regex to catch advertisements etc.
Option 1) Add a multi value NIP for relay side filters. Anything caught doesn’t get sent to the client. It could then sync to all other clients through your profile.
option 2) add the feature to clients directly. They’d still download the content but it would be filtered out before everything else was shown.
No single thing will block everything but it might form part of a multi layered approach.
