Pretty sure thats optional. I was presented with an online form and it had several questions I didnt want to answer. I asked which were mandatory and they pointed out like half of them.
The online form wouldnt let you submit though. So I had to do a paper form. Totally misleading and now I always assume institutions are trying to add pork to their info garhering.
