Someone just shared with me a line from a policy at their workplace that basically says, "This process is non-negotiable. Any exceptions to this process must be documented."
If a process has no exceptions, but has a mechanism for allowing exceptions anyway... At what point do these exceptions just become a part of your process? Is there any integrity left in the process?
